Instructions for filling out the form in Microsoft Word.

The form must be downloaded to your computer and then filled out.

Save the form once as a blank form

Fill out the form then save each filled out form with a new name

Use a black form each time you begin to fill out a new form

Be sure to save each form with a unique name

Attach the saved filled out forms to an email and send them to fredharlan@verizon.net
